Due to the rapid rise in EBT & SNAP‑related fraud nationwide, Centaurus Merchant Services now requires enhanced verification documents for all new businesses seeking to accept Electronic Benefits Transfer (EBT) payments.
Additional Paperwork Required
Please compile the following documents before submitting your merchant application. Incomplete applications will be delayed until all items are received.
- Government‑issued Photo ID (owner or controlling officer)
- Personal Utility Bill (showing owner’s residential address, dated within 60 days)
- Business Utility Bill (for your operating location, dated within 60 days)
- Corporate Certification (Articles of Incorporation, LLC Certificate, or equivalent)
- FNS Letter (Food Stamp Authorization Number issued by USDA)
- Voided Business Check (pre‑printed with legal business name and routing/account numbers)
- Recent Bank Letter or Statement confirming account ownership and full routing/account details
- Prior Merchant Processing Statement (if available)
